Insurance & Billing

Before scheduling, we recommend contacting your insurance provider to confirm your coverage, including any referral requirements, visit limits, or reimbursement rules. Clients are responsible for understanding their own benefits.

NE Wellness does not bill insurance directly. However, we can support clients who wish to seek reimbursement through their insurance provider. Depending on your situation, this may include:

  • Motor Vehicle Accident (MVC) claims

  • Workers’ Compensation (WC) claims

  • Submitting a superbill for out-of-network reimbursement

  • Using HSA/FSA funds by submitting receipts to your plan administrator

Please review the sections below to learn more about each option.

Motor Vehicle Accident (MVC) & Workers’ Compensation (WC) Claims

NE Wellness may work with Motor Vehicle Accident (MVC) and Workers’ Compensation (WC) claims when a referral is provided and the claim is open, active, and authorized for care. Our team will help coordinate the necessary documentation to bill these claims appropriately.

Services Available

  • Private Acupuncture

  • Massage Therapy

  • Craniosacral Therapy

Getting Started

To begin treatment under an MVC or WC claim, please complete the following before your first appointment:

1. Complete the MVC/WC Waiver
This form collects claim and adjuster information required for billing. (Client must enter insurance information)
Click here to access the waiver.

2. Complete the Informed Consent Form
All clients receiving acupuncture, massage, or bodywork services must complete this form prior to treatment.
Click here to access the waiver.

3. Bring Your Insurance Card
Our front desk will make a copy for your file.

4. Provide a Credit Card on File
A card is required to hold your appointment. Missed or late-cancelled appointments will be charged according to our cancellation policy.
Click here to review the cancellation policy.

Important: Clients remain responsible for any services not covered, authorized, or paid by their claim. We also recommend confirming with your claim adjuster that the services you are seeking are authorized before scheduling.

Superbills for Out-of-Network Reimbursement

Superbills are available upon request for private Chinese Medicine/Acupuncture appointments.

A superbill is an itemized receipt including treatment codes and provider information required to submit a claim to your insurance provider for possible out-of-network reimbursement.

Please note: Superbills are not available for Community Acupuncture sessions, as these are offered at a reduced group rate and are not structured for insurance reimbursement.

If you would like a superbill, please notify the front desk when booking or prior to your appointment.

Payment for services is required at the time of your appointment. After your visit, you may submit your superbill directly to your insurance provider.

Because coverage varies by plan, reimbursement is not guaranteed. We recommend contacting your insurance company to confirm whether your plan includes out-of-network acupuncture benefits.

Questions to Ask Your Insurance Provider:

  • Do I have out-of-network acupuncture benefits?

  • Do I need a referral or diagnosis code?

  • What percentage of reimbursement is covered?

HSA / FSA Reimbursement

NE Wellness does not process HSA or FSA cards directly.

However, many clients are still able to use Health Savings Account (HSA) or Flexible Spending Account (FSA) funds by submitting an itemized receipt to their plan administrator for reimbursement.

After your appointment, our front desk can provide a receipt that includes the information typically required for HSA/FSA reimbursement.

Because eligibility is determined by your individual plan, we recommend confirming with your plan administrator whether your services qualify.
