• Fill out an MVC/ WC waiver prior to appointment. Click here to access waiver

• Fill out an Acupuncture/ Massage/ Bodywork/ Informed Consent Waiver. Click here to access waiver

• Bring insurance card for front desk to scan at first appointment

• Submit a credit card to be kept on file. Credit cards are required to hold the appointment. Clients who miss appointments will be required to pay cost of appointment out of pocket within the standards of our cancelation policy. Review our cancelation policy here.

Superbills for reimbursement

Superbills are available upon request for private appointments. We do not provide Superbills for community acupuncture. Please let the front desk know prior to the appointment if you would like a Superbill. Client must pay for appointment day of and can self submit their Superbill to their insurance company to seek reimbursement. Reimbursement is not guaranteed. We recommend that the client contacts their insurance company ahead of time to see if they are covered. Superbills are completed and sent out to clients monthly.

HSA/ FSA reimbursement

Client must pay for appointment day of and can submit a receipt to their HSA/ FSA account for reimbursement. Reimbursements are not guaranteed. We recommend that the client contacts their account representative ahead of time to see if they are covered. Receipts are auto emailed. If you would like a paper receipt you can request it from the front desk.
